Social, recreational and community participation through individual and group activities.

 

Eskleigh can support you to become more involved in a range of community, recreational and social activities.

The type of activities you get involved in are up to you and can be undertaken either as part of a group or individually, weekdays, weekends and evenings.*

Your coordinator can help develop a Community Access Plan that is aligned with your personal goals and funding.

Many of our participants choose to become involved in local community and sporting groups, social groups, library visits, participate in art and craft or enjoy day excursions.

Community access can also help support you to fulfil day to-day tasks such as attending a training session, shopping and banking or attending a doctors appointment.

All services are provided by our skilled Support Workers who offer friendly and professional support and have the necessary qualifications to ensure you get the most from your selected activities.

FlowPoint

All services are supported by Eskleigh’s cloud-based care portal, FlowPoint which gives you and your support network real-time access to all aspects of your support plan.

You can access and update details around funding, review calendars and appointments, communicate with your support workers, access important forms and keep track of your goals, and much more.

FlowPoint offers transparency, convenience and choice, whilst keeping you and your support network fully informed around all aspects of your care.
